Cerebrospinal fluid (CSF) cell counts are traditionally performed by manual microscopy using a hemocytometer. This procedure is time and labor-intensive, and requires experienced laboratory staff. Automated cell counters have been successful at alleviating some of these issues but many are not accurate at analyzing clear and colorless specimens with clinically relevant low levels of total nucleated cells (TNCs) and red blood cells (RBCs) because the instruments produce background counts that are higher than normal cell counts. Since most of the CSF specimens that enter the clinical laboratory have low cells counts in the normal range, laboratory staff must revert frequently to the manual method to obtain results for both RBC and TNC.1,2
